WordPress Submission Offline Documentation

I’ve seen many WordPress themes with a single html file linking to the full online documentation.

According to your recent experience with reviewers, is it still valid to add such single html or do I need to add the complete offline documentation?

Is it required to keep all media queries grouped at the bottom of the stylesheet? Or is it more practical to separate the code by features with its own media queries?

If you are talking about wordpress theme then:
Themes must have documentation available online that is publicly accessible. So, a single html file linking to the full online documentation is fine but you can include offline documentation also.
