What is Release forms? ZIP or JPEG/PNG?

Hi guys. Please help!!! I’m new here. Video project is not being sent because of the Release Form field. When I download a ZIP - it requires a JPEG; when I download a JPEG - it requires a ZIP. I’m desperate.
