PSD Template approved but HTML Template got Hard Rejected

My HTML template got hard rejected could somebody point out the lacking points in my HTML version?

I am aware about this “Getting an approval for PSD doesn’t guarantee that you will get approval for HTML and as the same HTML template doesn’t guarantee that you will get an approval for WordPress ( or any CMS )”, so I need some suggestions to improve in my HTML template so that I can resubmit it.
