I want to use stock photos from shutterstock, with images of people, in the pictures that I want to buy from shutterstock. What are the rules around using images of people in WordPress theme demo content / uploaded images in theme description?

Depends on the Shutterstock license details.

Assuming they are not celebrities then I can’t imagine it is likely to be a big issue to use these as part of your demo but you need to make sure there’s no restriction in the license around commercial use etc.

100% do not include them in the downloadable version that buyers get. In here replace them with placeholder images.
