Hey - we have the Envato Elements subscription and would like to use a sound clip for our business hold music - it says that it is Royalty Free so do we need to pay any extra for PRS and PPL?

You want to address this question to Elements support, only they can give you a binding answer.
The Royalty free term does not mean the music is free of Performance Royalties. Whether there is anything to pay to PRS depends on their own policies and process regarding “on hold” music, for which Envato cannot give you any guidance.
