My question if I download this version, do i Need to start from scratch with my current website?
is everything gone? How does this works.

If your website activated child theme then theme update should not effect on your website customization changes made by you. If you are activated parent theme then theme update to latest version your custom work will loss. Your database mean site content should not effect.
If you like You can contact the author of your purchased theme and ask them about it.
