I’m looking for a survey script that works like survey monkey or qualtrics that’s written in either PHP or JS. The response data needs to be stored in either a database (accessible with a backend) or sent as JSON.

Can some give me some suggestions?

Lime Survey tool written in PHP. It allows you to create, conduct, and analyze surveys and can store data in a MySQL database. It also has options to export data as JSON if needed.
