Wordpress plugin for Custom contact forms

Hi,
I’ve got a Wordpress site with about 10 users (agents) and a single contact form that they fill in when they need to book a service, at the moment the form has about 20 fields and about 3 quarters will always be the same, say: Agent Name, Agent Phone, Agent Address… so they have to fill in a lot of fields that aren’t really necessary…
I was wondering if theres a plugin that might allow for users to have custom forms (pre filled forms) so then they only enter DATE IN and DATE OUT…

? Any clues?
