Sumo Reward Point "Refer a friend form" not showing

Hi,
I am trying to implement Sumo Reward Points plugin in my website. However, for some reason, I am not able to show the “refer a friend form” in a customer’s dashboard, although it is enabled in the settings. I have attached below some images to demonstrate my setting in the backend and account page in the front end. Do you have any idea why might this be happening? or any possible solutions to this problem. Many Thanks
