Submit Form With Dependant Dropdown Lists / Menus

I haven’t been able to find something like this but perhaps I’ve overlooked it. If there isn’t one available, I’m certainly willing to pay someone to make it. I’m looking for a plugin WordPress form that has the capabilities of dependent drop down lists for submitting.

Ideally, Something with a couple of fields for contact information then some drop downs that they can be click through to specify exactly what product they have.

For Example:

First List Contains Condition: New Or Used

Second List Contains Brand: Brand 1, Brand 2, Brand 3, Etc

Third Contains Model Type: Model 1, Model 2, Model 3, Etc

Also, Have the ability to list one product or multi products in the same form depending on how many the user has.
