Hello, I am trying to find a PHP form builder that allows me to easily create forms and then export the script. Im looking for one that does this independently of the app. Meaning, when I design a form I can take that code and use it on a site without the app. Second, I am looking to connect the form to submit to a MS SQL/MySQL database, preferably MS SQL. I found the following app that looks like it meets my needs but it doesn’t look like the author is responding to anyone. Does anyone use this app that can tell me if it will work for me, or know of any others that would?
