I am making video using the template video and audio here in envato for my youtube channel intro. Do I need to create a separate license everytime I upload a single video on youtube using the intro I created? I mean my subscription here might end already soon and I will still be using the intro I made in the future. Please help me, I really need some answer. Thank you guys!
Yep, every time unless author whitelists your whole channel. Just ask him.
About the license after subscription end - I think it is ok but you should read terms or contact support to confirm.

Search for the author in AudioJungle (not Elements) and there should be a contact form on the authors profile-page.
