Support Dashboard

Hi All,

I’m looking for a support dashboard with these features:

  1. Customer submits form from front end
  2. Entries can be viewed in back end in table form.
  3. Reply to customer in backend, and it shows if there was a reply or not.

I know there are a lot of support/ticket systems out there, but I cannot find one where I can integrate a custom form to the ticket system.

Hope this is clear enough.
