Someone can give advice about how to add my template correctly?

The problem is that, 2 times they have rejected my work, now I don’t know how to add, so they accept, I’m adding templates for the first time, may be that is why i missed and missin’ some key points.
Pls help if that is possible!

What was the message you are getting back?

If it is getting reviewed then you are probably submitting correctly and the issue is with design or code.
