Script for Customers Document Management

Well I am looking for a solution for an agency.

They need a script to collect reqired documents from their customers.

It should be like a “form builder” but not to send informations, just to collect and reaching it to the agency to check if the documents are correct.

  1. The customer should see a dashboard after loging into the system.
  2. Then see empty fields to fill like name, surename, phone, birthdate and most important: see empty “upload” attachment fields to upload a “x count” of required documents .
  3. After filling the form, and uploading required documents, the user should be able to see a status behind each document on his profile. Like: “unread”, “in process”, “approved” and “declined” (so he can reupload it).

Is there something like that in the store?

That is custom made thing.
