Registration Form - Plugin?

Anyone know where to get a online registration plugin? I see lots of plugins for simple registration but I am looking for a detailed one that would be used for someone to open a bank account.

I need it to collect certain amount of information that would be based from the actions of certain options selected:

Example: If a person wanted to open a personal account Vs a corporate account. The type of information that needs to be collect will be different between those two options.

I used to subscribe to Adobe Forms and they did all this but discontinued this service.

Honestly, for a banking website, I’d code my own forms to ensure optimal security and functionality.

For such a service, I’d be concerned about the security and use of the form.

Not something I’d depend on a third-party resource (such as a plugin) for. Just my two cents worth.
