I have problems to install medicale wp theme. This theme recommends the following plugin: TinyMCE Advanced, active Revolution Slider and update the WPBakery Visual Composer.
Do I need to buy WPBakery Visual Compose license?
No you don’t
If they are bundled then you don’t get a license code to auto update but it will work regardless.
The author of the theme can release updates to both theme and plugin in the future

For theme specifics you need to ask the author for support (also check documentation)
There are a few simple ways to check if your theme includes Visual Composer, but you should ask your theme author about these. Also you should check your plugin area to see if maybe Visual Composer is installed or not …
