Need clarification on recurrent Soft Reject issues for HTML Template

We have some recurrent issues regarding a HTML template. We’ve been trying to get a HTML template approved. We’ve done several re-uploads after reviewing the soft reject reasons and making the thought necessary changes, but some of them keep repeating themselves and we can’t figure what we’re not doing right or missing. These are the issues:
