This is my animated entry, the first preview is included in the template and the second one was made to illustrate editing possibilities, I kept the design as light as possible but you can replace all colors, text or add images, the gif will get bigger but the possibilities are endless.
Please keep in mind that this template won’t animate on all versions of Photoshop; I have only tested it on Photoshop CS6 Extended x64.
