License questions / License on php comments / Extended License

All my php files starts with:

According that I want to sell exclusively on codecanyon, what should I mention in the beginning of each php file?

And what licence file should I insert in the package of my script?

I don’t want to offer an “Extended License”, is that possible? How?

yes you can turn off extended license from your dashboard.

Thank you for your reply. Indeed, I could turn off the extended license from my dashboard. I looked at the regular license of Envato community at https://codecanyon.net/licenses/terms/regular
The point 5 is perplexing to me:
5. You can make any number of copies of the single End Product, as long as the End Product is distributed for Free.

Does that mean that if I redistribute for free my product, then clients can redistribute for free or does that mean clients can redistribute my product even if my product is not free?

In others words: Is the End Product my product or is the End Product the product of the client who paid to download my product?
