You should be able to setup something similar with Advisor Quiz:

This plugin includes support for multi-page quizzes with an option to recommend products to a user based on their answers. Here’s an example from the plugin demo:

But if you want the look of the quiz to also visually match the example you linked on Prose it will need some styling. If you’re not comfortable with custom code then I would suggest checking with the developer to see if they’d be able to help you customize the design.
