I have uploaded different items and every time hard rejection placed I don’t know why? please any one guide in this matter.

which demo url?
A demo url or a preview of whatever it was that you submitted and that was rejected
I didn’t have demo url I upload it without it
Then that’s the first problem - it won’t have been reviewed without that as it’s a requirement.
Envato don’t host your site - they just provide a way of displaying your demo (which is your responsibility)
Still impossible to say if it would then be approved or rejected but without a working demo you have zero chance.
how to upload a demo of mobile app or website, as i have create index file separately but didn’t upload it.
You need to get your own hosting and domain and upload the template there and include that demo url in your submission
