Can I use a github plugin to create a HTML template if the plugin author has not provided license information?
To avoid any issues I’d contact the github repo owner and ask them directly.

if the plugin has permission for coomercial use, redustribute abd resell permission as like MIT license then you can otherwise you can’t use that plugin to make your end product for sell. Much possibility to get Copyright issues.
