Feedback Request: My HTML Quote Form Got Rejected — Need Help Improving

Hi everyone,

I just submitted an HTML delivery quote form I created under the name Quotive, but it was rejected by the Envato review team. They encouraged me to post here in the forums to get some feedback from the community.

This is my first submission, and I’d really appreciate any tips, suggestions, or critique on what I can improve—whether it’s design, code structure, user experience, or anything else.

HTML? Seems too basic…

Check the items already existed in the marketplace to see what kind of features they have but it still doesn’t guarantee approval.

You will need to bring something new, otherwise, the item will be most likely rejected.

Demo execution is weak too - al sorts of problems with typography, margins, inconsistency, form is too big etc.
